Michael J. Lipp is the Chief Medical Officer at the Center for Medicare & Medicaid Innovation (CMMI) and leads the clinical activities across CMMI models and groups. Prior to joining CMMI, Dr. Lipp held various roles in the private sector, which included leading providers from fee-for-service to value-based care and developing clinical programs that drive quality and reduce costs. He was previously Vice President of Clinical Transformation at Healthcare Partners, where his activities included leading a primary care network and a house calls program for high risk patients. Dr. Lipp also served as the National MSO Medical Director for ChenMed, and he was an Assistant Professor of Clinical Medicine at Columbia University in New York. Dr. Lipp is board certified in both Internal Medicine and Gastroenterology.

She works directly with the Board of Directors and the CEO to develop the federal strategy She also serves in a limited role as the Vice President of Public Payor Health Strategy in the Care Coordination Institute at Prisma Health in SC. She previously worked as Senior Advisor and Group Director at the Center for Medicare and Medicaid Services Innovation (CMMI) and as the Chief Executive Officer of several systems including Bon Secours, SSM Health, and CaroMont Health. Verne Rinker serves as a health information privacy specialist with the HHS Office for Civil Rights (OCR). He leads OCR’s administration and enforcement for the Patient Safety and Quality Improvement Act of 2005 in addition to various policy aspects of the HIPAA Rules. He also serves as the OCR representative to the HHS Privacy Incident Response Team. He has served in several key roles on OCR’s HIPAA Privacy team. Areas of work have involved outreach, enforcement, HIT, emergency preparedness, HIV/AIDS, and the HIPAA audit program. Prior to working for OCR, Verne served in the Medicare Contractor Management Group in the Centers for Medicare & Medicaid Services (CMS), where he assisted with development of a new Medicare Contractor Strategic Plan. He served as a member of the CMS Privacy Review Board. Verne began his career in late 1999 as a Presidential Management Intern with CMS. Through the Presidential Management Intern program, he served in the HHS Office of the General Counsel and at the Office of Management and Budget.

Previously, Adam was a regulator at the U.S. Department of Health and Human Services, where he played a fundamental role in administering and enforcing the HIPAA rules. As a CISSP, Iliana works hard to bridge the gap between legal requirements for the security of health data and security industry best practices, so that clients can better understand data security issues and jargon. Paloma Izquierdo-Hernandez is the President and Chief Executive Officer of Urban Health Plan (UHP), a network of community health centers located in the South Bronx, Central Harlem and Northwestern Queens. From its humble beginning as a one site local community health center, Urban Health Plan is considered a significant economic anchor and engine that has contributed to the resurgence in the growth and development of the local Bronx area. Ms. Izquierdo-Hernandez has devoted her career to reducing the health disparities in the Bronx community which is located in what has been identified as the poorest congressional district in the country. This commitment is reflected in the many achievements and successes of the organization under her leadership. With fierce determination, she shepherded the expansion of Urban Health Plan from a one-site facility in the South Bronx, to a network of federally-qualified community health centers that includes ten sites in the Bronx, Central Harlem and Queens; eleven school health programs; a mental health facility; two part-time facilities for at-risk populations; three WIC programs; and multiple other grant-funded programs.

Dr. Mate oversees IHI’s research program focused on developing and scaling models for implementing high-quality, low-cost care both in the US and in international settings. An internal medicine physician, Dr. Mate is also Faculty in the Department of Medicine at Weill-Cornell Medical College. Terry Fulmer is the President of the John A Hartford Foundation in New York City, an organization dedicated to improving the care of older adults. Established in 1929, the Foundation has a current endowment of over $560 million and is world-renowned for philanthropy devoted exclusively to the health of older adults. She serves as the chief strategist for Foundation giving and is also the chief spokesperson for advancing the Foundation’s mission. Dr. Fulmer is nationally and internationally recognized as a leading expert in geriatrics and is best known for conceptualization and development of the national NICHE program and research on the topic of elder abuse and neglect, work that has been funded by the National Institute on Aging and the National Institute of Nursing Research. Her recent effort with the Age-Friendly Health Systems initiative in partnership with IHI is a potential game changer for how we think about care for older adults.

Carol Raphael is a senior advisor at Manatt Health. She served as chief executive officer and president of the Visiting Nurse Service of New York (VNSNY), the largest nonprofit home health agency in the United States, for over 20 years. Under her leadership, the organization launched innovative models of care and health plans for complex populations with chronic illness and functional impairments. Prior to joining VNSNY, Carol held executive positions at Mt. Sinai Medical Center and in New York City government. She also served as board chair of AARP. Carol is regarded as a national expert on post-acute care and long-term services and supports (LTSS) policy, payment, workforce and technology. She has considerable experience developing new business models, restructuring clinical services and shifting care to home and community-based settings. She is the chair of the Long-Term Quality Alliance, an alliance of payers, providers, consumers and policymakers working to strengthen and better integrate the LTSS system. She was the chair and remains a board member of the New York eHealth Collaborative, a public-private partnership building a health information exchange platform. She helped launch a Commonwealth Fund project to spur the development of high-performing integrated health plans for a dually eligible population.

Susan Dentzer is the Senior Policy Fellow for the Robert J. Margolis Center for Health Policy at Duke University. She focuses on health system transformation, biopharmaceutical policy, and other key health policy issues. Dentzer is one of the nation’s most respected health and health policy thought leaders and a frequent speaker and commentator on television and radio, including PBS and NPR, and an author of commentaries and analyses in print publications such as Modern Healthcare. She was also the editor and lead author of the book Health Care Without Walls: A Roadmap for Reinventing U.S. Health Care. Previously, Dentzer was President and Chief Executive Officer of NEHI, the Network for Excellence in Health Innovation, she was senior policy adviser to the Robert Wood Johnson Foundation, and before that, was the editor-in-chief of the policy journal Health Affairs. She was the on-air Health Correspondent for the PBS NewsHour. Dentzer wrote and hosted the 2015 PBS documentary, Reinventing American Healthcare, focusing on the innovations pioneered by the Geisinger Health System and spread to health systems across the nation. Dentzer is an elected member of the National Academy of Medicine (formerly the Institute of Medicine) and serves on the Board on Population Health and Public Health Practice of the National Academies of Science, Medicine, and Engineering. Linda Blumberg is an Institute fellow in the Health Policy Center at the Urban Institute. She is an expert on private health insurance (employer and nongroup), health care financing, and health system reform. Her recent work includes extensive research related to the Affordable Care Act (ACA); in particular, providing technical assistance to states, tracking policy decisionmaking and implementation at the state and federal levels, and interpreting and analyzing the implications of particular policies. Examples of her work include analyses of the implications of congressional proposals to repeal and replace the ACA, delineation of strategies to fix problems associated with the ACA, estimation of the cost and coverage potential of high-risk pools, analysis of the implications of the King v. Burwell case, and several studies of competition in ACA Marketplaces. In addition, Blumberg led the quantitative analysis supporting the development of a “Road Map to Universal Coverage” in Massachusetts, a project with her Urban colleagues that informed that state’s comprehensive health reforms in 2006. She serves on the Cancer Policy Institute’s advisory board and has served on the Health Affairs editorial board. From 1993 through 1994, she was a health policy adviser to the Clinton administration during its health care reform effort, and she was a 1996 Ian Axford Fellow in Public Policy.
